Nurture Health and Care Wisdom Academy launches IAFN's SANE programme

Dear Colleagues

Nurture Health & Care Wisdom Academy are pleased to share details of our upcoming Sexual Assault Nurse Examiner (SANE) Adult / Adolescent & Paediatric Programme, delivered through the Nurture Health & Care Wisdom Academy.

This UK-based programme aligns with the International Association of Forensic Nurses (IAFN) standards and is designed for nurses and midwives working within SARCs or forensic healthcare services who wish to develop specialist forensic examination expertise in a trauma-informed, psychologically safe learning environment.

The programme includes:

64 hours of IAFN-aligned teaching
16 hours Clinical skills laboratory training
Supervised clinical practice and preceptorship
Logbook-based competency sign-off, including hours of practice
Preparation toward SANE-A / SANE-P certification eligibility

UKAFNP CPD Funding Now Open

Start 2026 with support for your development—apply for up to 50% course funding (max £1,000)

We’re excited to launch Round 1 of UKAFNP CPD & Academic Funding —a tangible demonstration of our commitment to members’ ongoing learning and career progression across police custody and SARC practice.

Funding is supplementary (up to 50% of fees, capped at £1,000 per application) and is intended to help you access recognised courses, modules, and events aligned to forensic healthcare. Travel and accommodation are not normally funded unless part of a residential course of three days or more.

The application form and full guidance are available to members —complete and submit online via the document downloadable at the bottom of this page.

Who can apply (at a glance)

Current UKAFNP member ; Registered Nurse/Midwife/Paramedic (NMC/HCPC).
Minimum service normally 2 years (including agency ≥0.5 WTE) with a brief career summary.
Co-funding required (employer and/or personal); UKAFNP is not a full funder.
Course relevance : clearly linked to custody or sexual offence examination practice and accredited by a UK HEI or recognised professional body.
One award per calendar year (max £1,000).

You’ll be asked to provide: registration details, a five-year career history, course information and objectives, how learning will impact practice and patient care, evidence of co-funding , and a manager/line-manager reference. Shortlisted candidates may be invited to an MS Teams interview . Successful applicants agree to share a short impact summary/resource within six months.

30 May – Shortlist agreed
UKAFNP Panel reviews applications.
30 Jun – Funding update
Chair of Education Forum issues confirmation emails to Round 1 applicants.

31 Jul – Reimbursement details due
Applicants submit required evidence of co-funding .
31 Aug – All payments completed
UKAFNP processes payments.

Tips for a strong application

Show impact : Be specific about how the course will improve your clinical practice and patient outcomes in custody/SARC settings.
Evidence co-funding early : Include your employer confirmation or proof of personal payment intent.
Choose eligible courses : UK HEI/professional-body accredited; within the scope of custody/SOE practice.

FCN SARC Network News — Issue 18: Key updates for forensic healthcare

The latest Forensic Capability Network (FCN) SARC Network News – Issue #18 (18 November 2025) is out now, with several practical updates for teams working in Sexual Assault Referral Centres and related forensic settings. (Link to full newsletter will be added.)

Webinar: Understanding ISO 18385 – Forensic DNA-grade consumables

When: Wednesday 3 December, 10:00–12:00 (MS Teams, registration via Eventbrite)
This two-hour session covers what constitutes DNA-grade consumables, how they’re manufactured, EtO treatment vs batch testing, and what to look for in records, QA certificates and product labelling. Attendees receive a slide pack to use as a local training toolkit.

Using national validations — FCN asks for your feedback

Working with SARCs across England and Wales, FCN has delivered national validations for SARC consumables (FCN-SAR-VAL-0006) and forensic cleaning (FCN-SAR-VAL-0012) , plus guidance to help sites verify that validated processes are fit for purpose in their own facilities (e.g., FCN-SAR-VER-0002 Cleaning Verification and FCN-SAR-GUI-0036 End-to-End Verification – National Roll-Out ). FCN invites suggestions on how to improve validation/verification approaches.

Current national stance on ‘self-swab rape kits’

The FFLM has written to universities in England and Wales regarding the promotion/distribution of self-swab kits to students and has shared its stance publicly; the AFSP Body Fluid Forum has also issued a position statement. Contact FFLM for further information.

SARC FSR Code compliance survey — 98% response rate

FCN reports a 98% response rate to the September survey. Future editions will share anonymised trends from UKAS assessments to help reduce findings; the OFSR supports the approach. The next update survey goes out in December; non-responders will be monitored by the Regulator. Sites are encouraged to share reports, raise concerns about disproportionate findings, and nominate the correct contact for future returns.

Where Two Worlds Collide: HCPs at the Crossroads of Law and Clinical Practice

Forensic Healthcare Practitioner Week (#FHPWeek2025) returns from 3rd to 9th November , celebrating the vital work of Forensic Healthcare Practitioners (FHPs) across the United Kingdom.

Each year, UKAFNP proudly recognises the diverse and essential contributions of nurses, paramedics, midwives, and doctors who deliver care within police custody suites and Sexual Assault Referral Centres (SARCs). These practitioners stand at the unique intersection of health and justice , safeguarding the well-being and rights of some of society’s most vulnerable individuals.

This year’s theme, “Where Two Worlds Collide: HCPs at the Crossroads of Law and Clinical Practice,” builds upon the success of the 2025 UKAFNP Conference. It invites reflection on how forensic healthcare professionals navigate complex environments where clinical ethics, legal frameworks, and public service meet.

Throughout the week, UKAFNP will:

Celebrate the expertise, compassion, and dedication of FHPs across the UK.
Raise awareness of the specialised skills required in forensic healthcare, including documentation, evidence collection, safeguarding, and trauma-informed care.
Promote recognition of the value FHPs bring to both the NHS and the justice system, particularly in ensuring fairness through the giving of evidence and expert clinical opinion.

The PGCert Advanced Forensic Practice for Health Care Professionals is open to applications for the Janaury 2026

The University of Greenwich has developed a PGCert Advanced Forensic Practice for Health Care Professionals course, allinged with UKAFNP ASET Award. Many of you have enquired about their brand-new programme. Sadly, they didn't have enough people signed up to run the course in September, so thy are having one more attempt: January 2026.

They already have some people signed up, from Custody and Sexual Offences' Services, but we require more to make this a viable cohort. This UKAFNP-inspired programme is designed for healthcare professionals working within the forensic arena, offering two distinct routes: Custody and Sexual Offences.

Programme Overview

This innovative and comprehensive PGCert aims to provide healthcare professionals with advanced knowledge and skills in forensic practice, preparing them to excel in both custodial and sexual offences settings. Aligned with the Advanced Standards for Education and Training (ASET) of the UK Association for Forensic Nurses and Paramedics (UKAFNP) and endorsed by the Faculty of Forensic and Legal Medicine, the programme focuses on a multidisciplinary approach to forensic healthcare.

The programme consists of three key modules:

Introduction to Advanced Forensic Practice (15 credits)
Delivered by the School of Health Sciences, this module introduces key principles and practices in forensic healthcare.
Legal Skills for Health Care Professionals (15 credits)
Taught by the School of Law and Criminology, this module equips healthcare professionals with essential legal knowledge relevant to forensic practice.
Clinical Competencies for Care in Custody / Sexual Offences Services (30 credits)
A deep dive into clinical practices in custody or sexual offences services, delivered by the School of Health Sciences. This module focuses on honing clinical skills and achieving key competencies essential for practice.

Programme Structure and Delivery

The first cohort is expected to start in September 2025. The curriculum will be delivered through a hybrid model, combining on-campus learning at Avery Hill and Greenwich Maritime campuses, with online live sessions and self-directed study. Students will benefit from a rich educational environment led by expert educators from diverse fields such as advanced clinical practice, mental health, criminology, and forensic science. Teaching will include state-of-the-art forensic simulation, digital literacy, and innovative learning methodologies to ensure students receive an outstanding academic experience.

Key Component: UKAFNP Competencies Document

A unique aspect of the PGCert programme is the incorporation of the UKAFNP Competencies Document. This work-based learning record allows students to demonstrate their knowledge, skills, and attitudes while reflecting on key forensic healthcare issues. The completion of these competencies, under the guidance of an approved mentor, is essential to achieving the UKAFNP ASET certificate.

Non-Sterile Alcohol Free Wipes – Important Safety Notice Update

UPDATE!

To follow up on our earlier safety bulletin about the Sterowipe alcohol-free wipes used in SceneSafe 's blood collection and hygiene kits, we can now confirm that a replacement wipe has been tested by Eurofins Forensic Services (EFS) and shown to be drug and alcohol-free. It has been discovered that there is a low risk of naturally occurring bacteria being present in the Sterowipe, which may be a health risk.

Regarding SceneSafe kits in stock containing the Sterowipe component, please contact your Account Manager to provide the kit type, batch number, and quantity. SceneSafe will make arrangements to send replacement wipes to your organisation.

The outer packaging for the two wipes is very different. I've included below images of the outgoing Sterowipe, which should not be used, and the replacement validated ReliWipe.

DO NOT USE

To ensure kit users can remain operational, it has been agreed that existing pre-made stocks of kits held by SceneSafe that have been assembled with the Sterowipe will still be supplied. But with a replacement wipe supplied in a packet attached to each kit, along with instructions to use the replacement wipe.

From 16th September 2025, all new kits manufactured by SceneSafe will now contain the “ReliWipe” sterile alcohol free wipe, and this can safely be used for all skin surface preparation work. All kits from this date will have their labels updated with the added text “UPDATED SPEC.” which will be printed to the right of the batch control number.

If you are in any doubt about whether an existing kit in your possession has the correct wipe, please don't hesitate to get in touch with SceneSafe and give them the kit name and batch number and we can check our records to confirm its usability.

Alcohol/Drug Blood Testing Kits – Important Safety Notice

Risk with non-sterile alcohol free wipes

The UK Health Security Agency (UKHSA) and the Medicines and Healthcare products Regulatory Agency (MHRA) have advised that certain non-sterile alcohol-free wipes should no longer be used, after an investigation confirmed that some wipes may be contaminated with Burkholderia stabilis.

Burkholderia stabilis is a naturally occurring bacterium. It rarely causes infection in healthy individuals and the overall risk to the general population is very low. However, immunocompromised individuals and those with risk factors such as cystic fibrosis, or with intravenous lines in place, are at greater risk of developing infection.

Since 2018, there have been 51 confirmed cases in the UK suspected to be linked to affected non-sterile alcohol-free wipes. No deaths have been reported, but some infections required hospital treatment. On this basis, UKHSA/MHRA guidance is that non-sterile alcohol-free wipes must not be used for skin preparation prior to taking blood samples.

This affects all users of SceneSafe Alcohol / Drug Blood Collection Kits , as the non-sterile alcohol-free wipe supplied in the kits has been identified as one of the products possibly affected.

Action required:

With immediate effect, please instruct all healthcare practitioners not to use the non-sterile alcohol-free wipe supplied in the kit .
Discard this non-sterile wipe and use an alternative sterile alcohol-free wipe .
All other kit components remain unaffected and should be used as normal.

SceneSafe is working closely with Eurofins Forensic Services to validate suitable sterile alcohol-free replacement wipes. We expect this work to conclude within days and will contact you again once replacements are approved. Additional stocks of sterile wipes will also be made available to purchase as a standalone product.

Interim guidance:

Clearly there is a requirement to remain operationally effective. The Faculty of Forensic & Legal Medicine has stated that, unless the skin is visibly dirty or contaminated, blood can be taken without skin preparation. However, we recognise this may not align with your current operating procedures. Following advice from specialist forensic consultants, our interim recommendation is that if a wipe is to be used, it must be a sterile alcohol-free wipe.

Dr Marc Bailey Named Forensic Science Regulator

The UK Association of Forensic Nurses and Paramedics (UKAFNP) welcomes the announcement made on 21 July 2025 by Dame Diana Johnson , Minister of State for Policing and Crime Prevention, confirming the appointment of Dr Marc Bailey as the new Forensic Science Regulator .

Dr Bailey brings with him a distinguished background in science and regulation, having held senior leadership roles at the Medicines and Healthcare products Regulatory Agency (MHRA) , including Chief Science, Research and Innovation Officer and Head of Analytical and Biological Sciences .

His three-year term as Forensic Science Regulator will begin on 26 July .

This appointment follows a competitive public process in accordance with the Governance Code on Public Appointments .

�� Regulator’s Notification 01-2025: FSA-BIO 100
The Forensic Science Regulator has confirmed that the forensic medical examination of complainants will come under the statutory code from 2nd October 2025 . This will bring updates to guidance documents and declarations, with a strong focus on managing DNA contamination risk and demonstrating competency in the interpretation and presentation of expert opinion.

�� SARC End-to-End Verification
The FCN has published national guidance to help SARCs complete local verification of their DNA recovery methods. This includes templates, acceptance criteria, and support for SARCs to demonstrate their practices are fit for purpose. This work is supported by findings from recently published scientific papers in the Journal of Forensic and Legal Medicine.

�� Validation Tracker for SARCs
A new tool is being developed to help SARCs track and share progress on validation and verification activities, including cleaning methods and forensic practices. This is part of a wider initiative to improve collaboration, reduce duplication, and streamline compliance efforts across the sector.

�� Proficiency Testing Development
While the initial working group has completed its scope, FCN will continue to support the development of future SARC proficiency testing via the FINDS network. Interested participants are invited to get involved.

�� New Tool for Bruise Imaging on Darker Skin
The Metropolitan Police Service has trialled a new handheld device using cross-polarisation technology to enhance the visibility of bruising, especially on darker skin tones. Early results are promising, and this innovation could significantly improve injury documentation and support justice outcomes.

FSA-BIO 100 covers the forensic medical examination of complainants. This FSA will be coming under the FSR Code of Practice in October 2025.

The Forensic Science Regulator notification has been issued to make the forensic science community and criminal justice system aware that the Forensic Science Activity ( FSA ) for forensic medical examination of complainants will proceed as planned, and will be subject to the statutory code in October 2025.

The regulator will be issuing further guidance in support of the regulation of the forensic medical examination of complainants.

Improving Understanding and Support in Forensic Healthcare

On World Autism Day, it's important for forensic healthcare professionals to reflect on their interactions with autistic individuals, particularly within police custody suites, to ensure they are treated fairly, respectfully, and effectively.

Understanding Autism

Autism, or Autism Spectrum Condition (ASC), is a lifelong neurodevelopmental condition affecting how a person communicates, interacts socially, and perceives the world around them. Approximately 1 in 100 people in the UK are autistic, making it crucial for forensic healthcare professionals to understand this condition clearly.

Dispelling Myths and Misunderstandings

Autism is not a mental illness, nor is it something a person can outgrow. Terms like "high-functioning," "low-functioning," "Asperger's syndrome" (now simply referred to as Autism Spectrum Condition), or "autistic tendencies" are outdated or misleading. Autism presents differently in each individual, and assuming capability or needs based on appearance or superficial interactions can be detrimental.

Recognising Autism in Police Custody

Forensic healthcare professionals should be alert to potential indicators of autism:

Difficulty with social interaction or maintaining eye contact
Literal understanding of language or instructions
Sensory sensitivities (e.g., noise, lights, touch)
Anxiety or distress in unfamiliar environments
Repetitive behaviours or focused interests

Awareness of these indicators enables professionals to adapt their communication and behaviour effectively.

Supporting Autistic Individuals in Custody

The justice system, particularly police custody, can be highly stressful for autistic individuals. Implementing support strategies can significantly improve their experience:

Clear Communication : Use direct, simple language. Avoid metaphors, sarcasm, or abstract concepts.
Reducing Sensory Overload : Provide a quieter, calmer environment wherever possible. Adjust lighting and reduce noise.
Patience and Respect : Allow extra time for responses and avoid pressuring individuals during questioning.
Autism Passport Scheme : Encourage the use of autism passports or alert cards, which quickly inform professionals about an individual's specific needs.

You are supporting some of the most vulnerable, chaotic, and disenfranchised members of society, often at incredibly low and emotional points. This work is far from easy, but your attention to detail and compassion shine through. Thank you for all you do.

For those who celebrate, Christmas is a time for family, loved ones, and indulgence—sharing meals, laughter, and cherished moments with friends. However, for many of us in forensic healthcare, this time of year brings an intense workload as the rest of the UK slows down. This requires significant sacrifice, often missing out on celebrations to ensure we are there for those who need us most. Reflecting on the past year, I want to extend my sincere thanks to the UKAFNP Steering Group for their incredible efforts. While we’ve said goodbye to some familiar faces, we’ve welcomed new members brimming with fresh ideas and enthusiasm. Change can be challenging, but I am proud of how the team has embraced it.

This year saw some significant milestones:

We transitioned from UKAFN to UKAFNP across our website and communications, a move requiring extra steps from you, our members. By May, we aim to complete the transition to the new website and payment system.
We launched a new website, which we hope you’ve had a chance to explore.
#UKAFNPConf24 was a tremendous success, with attendees reporting transformative experiences from an engaging and informative day.
Behind the scenes, the UKAFNP team continues to work tirelessly to elevate the profile of forensic healthcare practitioners, ensuring our voices are heard in key conversations.

Celebrating the 2024 Julie Tekin Award Recipients: Keeley Roe and Sarah Fry

Recognising excellence in forensic healthcare

We are thrilled to announce this year's recipients of the Julie Tekin Award, a prestigious accolade that celebrates outstanding contributions to forensic healthcare. The award recipients were unveiled during the UKAFNP Conference on Friday, 20th September 2024, and we are proud to highlight the exceptional work of Keeley Roe and Sarah Fry , who have demonstrated exemplary commitment to the fields of Sexual Offence Examination and Police Custody Healthcare, respectively.

Sexual Offences Recipient: Keeley Roe

Keeley Roe, nominated by Donna Dabell (SARC Manager), is an exceptional member of the team at Mountain Healthcare, where she serves as the Associate Head of Healthcare for Yorkshire and the Humber, as well as the North East. Keeley’s journey into forensic nursing is unique. From the outset, she knew forensic nursing was her true vocation in life—a rare passion, as most healthcare professionals discover this career later in their journey. Her prior experience in education and as a volunteer for Rape Crisis helped shape her dedication to the survivors of rape and sexual assault.

As Associate Head of Healthcare, Keeley’s influence spans seven Sexual Assault Referral Centres (SARCs) across the region. She played a pivotal role in establishing Mountain Healthcare's contract in the North East and continues to make significant contributions to forensic practice through her role as a board member for the Faculty of Forensic & Legal Medicine (FFLM). Her commitment to patient-centred care is at the core of her decision-making and teaching. Keeley’s tireless dedication includes covering rota gaps to ensure patients always have access to the services they need, as well as regularly attending conferences and education events to share her expertise with peers.

Keeley’s efforts to standardise forensic healthcare practices led her to develop the case review process, which has been rolled out nationally across Mountain Healthcare and shared externally through training. She has supported more than 2,500 patients throughout her time in forensic nursing and continues to influence best practices in the field. Keeley completed the FFLM Licentiate in 2019 and has published articles in the FFLM website, including one titled "A Day in the Life of an FNE," to be released in November 2023.

Her contributions to the field were recently recognised by the Queen herself, and we are honoured to celebrate Keeley as the Sexual Offences recipient of the Julie Tekin Award.

Custody Recipient: Sarah Fry

Sarah Fry was nominated by Sandra Horton for her work transforming the way training and development are delivered at Mitie Care and Custody Ltd, bringing in-house training to the forefront and making significant improvements in both quality and efficiency. Since taking on the role of Training Manager, Sarah has not only improved the number and variety of training courses but has also brought about substantial financial savings.

One of Sarah’s most significant achievements is securing Level 7 Masters accreditation for the Forensic Medicine in Custody Healthcare course at Teesside University, which was previously accredited at Level 4. This 6-day course is a comprehensive program that covers all aspects of the healthcare professional’s role in custody, ensuring that staff are equipped with the knowledge and skills they need to excel.

Sarah’s innovative approach to training doesn’t stop at traditional methods. She is actively working towards integrating Virtual Reality (VR) into training, providing immersive experiences that help new staff better understand the role of healthcare professionals in custody settings. With funding secured for VR headsets, Sarah’s vision for modernised training is soon to be realised across all contracts.

Her work extends beyond healthcare professionals to the training and development of Custody Detention Officers (CDOs), where she has worked closely with Custody Detention Service managers to develop tailored competencies that ensure CDOs are better prepared and knowledgeable early in their careers.

Sarah’s commitment to excellence is reflected in the company’s ILS (Immediate Life Support) training compliance, which reached an impressive 98% in December 2023—an unprecedented achievement for the organisation. By bringing ILS training in-house, Sarah has overseen the development of 3 Course Directors and 11 new ILS instructors, while using the cost savings to invest in state-of-the-art ILS manikins for enhanced training.

The Forensic Medicine in Custody Healthcare course at Level 7 allows staff to gain 30 credits towards a Forensic Healthcare qualification, further advancing their careers. Sarah’s drive to improve standards has not only enhanced training but also contributed to the personal and professional growth of staff across the organisation.

In recognition of her transformative work and dedication to improving forensic healthcare training, Sarah Fry is a deserving recipient of the Julie Tekin Award.

Announcing the Launch of the Post Graduate Certificate in Advanced Forensic Practice at the University of Greenwich

A new pathway for healthcare professionals in forensic practice, starting January 2025

The University of Greenwich is excited to announce the launch of a new Post Graduate Certificate (PGCert) in Advanced Forensic Practice for Health Care Professionals , set to commence in January 2025 (*subject to validation approval in November 2024). This UKAFNP-inspired programme is designed for healthcare professionals working within the forensic arena, offering two distinct routes: Custody and Sexual Offences.

Programme Overview

The programme consists of three key modules:

Introduction to Advanced Forensic Practice (15 credits)
Delivered by the School of Health Sciences, this module introduces key principles and practices in forensic healthcare.
Legal Skills for Health Care Professionals (15 credits)
Taught by the School of Law and Criminology, this module equips healthcare professionals with essential legal knowledge relevant to forensic practice.
Clinical Competencies for Care in Custody / Sexual Offences Services (30 credits)
A deep dive into clinical practices in custody or sexual offences services, delivered by the School of Health Sciences. This module focuses on honing clinical skills and achieving key competencies essential for practice.

The one-year programme will be offered twice a year, with September and January intakes. The first cohort is expected to start in January 2025. The curriculum will be delivered through a hybrid model, combining on-campus learning at Avery Hill and Greenwich Maritime campuses, with online live sessions and self-directed study.

Students will benefit from a rich educational environment led by expert educators from diverse fields such as advanced clinical practice, mental health, criminology, and forensic science. Teaching will include state-of-the-art forensic simulation, digital literacy, and innovative learning methodologies to ensure students receive an outstanding academic experience.

International Overdose Awareness Day: Responding Effectively in Forensic Settings

Ensuring high-quality care in challenging circumstances

As healthcare professionals working in forensic settings such as police custody suites and sexual assault referral centres, we are often exposed to high incidences of drug overdoses. On International Overdose Awareness Day , it’s essential to highlight the critical role we play in responding to these life-threatening situations with precision, care, and systematic approaches.

The Reality of Overdoses in Forensic Settings

While we are often presented with cases involving sedatives like diazepam or opioids like heroin, it’s vital to avoid biases and consider alternative causes of unconsciousness. Each patient’s life depends on our ability to respond effectively, regardless of the substance involved.

The A-B-C-D-E Approach: A Systematic Method for Managing Emergencies

In these high-pressure environments, the A-B-C-D-E approach (Airway, Breathing, Circulation, Disability, Exposure) is an essential framework. It allows healthcare professionals to quickly assess and manage emergencies, prioritizing care and enabling timely interventions.

Airway
Always address the airway first. For someone unresponsive or making snoring noises, immediate intervention is required. Begin with basic maneuvers like jaw thrust or head-tilt-chin-lift, progressing to adjuncts such as the nasopharyngeal (NPA) or oropharyngeal (OPA) airway if needed. In deeply unconscious individuals, an i-gel may be the most definitive airway device available in custody.

Breathing
Once the airway is secure, assess and manage breathing. Apply oxygen as needed, using a simple facemask or high-concentration facemask, and ventilate with a bag-mask if the patient is in critical hypoxia or respiratory arrest.

Circulation
With breathing optimised, move to circulation. Assess for signs of shock or hypovolemia and manage accordingly.

Disability
Assess the patient’s neurological status. If the patient is unconscious, place them in the recovery position and consider interventions such as correcting hypoglycemia or administering naloxone in cases of suspected opioid overdose.

Exposure
Finally, assess and manage exposure, ensuring the patient is kept warm and their dignity is maintained.

In forensic healthcare settings, it's crucial to maintain an open mind when assessing unconscious patients. While certain substances may be common in overdoses, each case must be approached without assumptions to ensure all potential causes are considered.

Today, the HM Prison and Probation Service (HMPPS) has reactivated Operation Early Dawn, a contingency mechanism designed to provide extra oversight of prisoner transfers between police, courts, and prison. This reactivation comes in response to the recent influx of individuals entering the prison estate, especially in the north of England, following significant disorder and an already crowded prison system.

Understanding Operation Early Dawn

Operation Early Dawn ensures that individuals charged and remanded in police custody are only summoned to a magistrates’ court once a cell in the prison estate is confirmed to be ready for them. This measure aims to manage the current capacity concerns within the prison system, particularly in the following regions:

North East and Yorkshire
Cumbria and Lancashire
Manchester, Merseyside and Cheshire
East and West Midlands

With this operation in place, individuals will remain in police custody suites until their court summons is issued, ensuring that a secure location is available should they be remanded to prison. The decision-making process is coordinated through operational assessments conducted each morning and throughout the day by the Prison Service, courts, and police.

The government has committed to keeping Operation Early Dawn under constant review, activating or deactivating the measure as necessary based on evolving capacity challenges within the prison estate.

Impact on Police Custody Healthcare Professionals

The reactivation of Operation Early Dawn brings with it several implications for healthcare professionals (HCPs) working in police custody suites.

Increased Workloads

Custody HCPs may be required to cover additional custody suites that have been set up or designated to hold individuals under this operation. With more detained individuals potentially remaining in custody for extended periods, there is a likelihood of increased workloads. HCPs must remain vigilant, ensuring that the care provided to these individuals continues to meet high standards and aligns with the care individuals can expect in the community.

Managing Frustrations

The longer-than-usual detention in police custody, coupled with the uncertainty around court appearances and potential remand to prison, may lead to increased frustration among detained individuals. HCPs should be prepared to encounter individuals who are understandably upset or anxious about the delay in their case progression. Which can inturn increase the risk of self-harm. Effective communication, empathy, and maintaining a calm environment will be crucial in managing these challenges and ensuring the mental well-being of detained individuals.

High Standards of Care

Despite the operational pressures and potential increase in detainee numbers, the quality of healthcare provided in custody settings must not waver. HCPs should continue to adhere to established protocols, ensuring that all individuals receive appropriate medical attention, mental health support, and any necessary interventions in a timely manner. HCPs must consider the impact of prolonged detention especially when consider the continuation of an individual's medication.

Conclusion

Operation Early Dawn is a strategic response to the current pressures on the prison estate in the north of England, but it also brings additional responsibilities for police custody HCPs. As this operation unfolds, it is essential for HCPs to stay informed, maintain high standards of care, to enable them to best meet increased demands.

Let’s get the UK thinking about drinking and its impact on our lives

As we mark Alcohol Awareness Week from 1st to 7th July, it's time to reflect on the pervasive role alcohol plays in our society and the significant harm it causes. Coordinated by Alcohol Change UK , this week is dedicated to raising awareness, campaigning for change, and encouraging a national conversation about alcohol use and its far-reaching impacts.

Understanding Alcohol Harm

Alcohol often takes centre stage in people's lives. It's promoted during favourite sports events, advertised on commutes, and featured in films and TV shows. We turn to it in times of celebration, commiseration, and coping. Yet, the harmful effects of alcohol on our health and wellbeing are profound and far-reaching.

From disrupting sleep and straining relationships to causing long-term health issues and premature deaths, alcohol’s impact is pervasive. This Alcohol Awareness Week, we are diving deep into what we mean by 'alcohol harm' and challenging the stereotype of alcohol as merely an individual's problem. Instead, we aim to highlight its broader implications on families, communities, healthcare workers, and emergency services.

The Broader Impact of Alcohol

Alcohol does not harm individuals in isolation. It affects families, communities, and public services. Recognising this, we must discuss alcohol's role in our society and how we can mitigate its harm through better policies and cultural shifts. With appropriate regulation, clearer labelling, improved support and treatment, and a culture prioritising people over alcohol, we can protect and enhance our shared public services.

Categories of Alcohol Use

Understanding the various levels of alcohol use is essential in addressing the issue comprehensively. Here are the key categories:

Non-Drinker: Individuals who do not consume alcohol.
Low-Risk Drinking: Consuming no more than 14 units of alcohol per week, spread evenly over three or more days.
Hazardous Drinking: Drinking more than 14 units but less than 50 units per week for men and 35 units for women. Approximately 24% of adults in England and Scotland drink at this level, with higher rates in less deprived areas.
Alcohol-Use Disorder: A pattern of drinking where men consume at least 50 units and women 35 units per week, further divided into harmful drinking and alcohol dependence.

Alcohol-Use Disorder: Harmful Drinking and Dependence

Harmful Drinking: This involves a pattern of alcohol consumption causing direct health problems, including psychological issues, accidents, and physical illnesses such as pancreatitis, high blood pressure, cirrhosis, heart disease, and cancers.
Alcohol Dependence: Characterised by craving, tolerance, and an obsession with alcohol, despite its harmful effects. It affects over 600,000 individuals in the UK.

Alcohol Withdrawal

Alcohol withdrawal occurs when someone dependent on alcohol abruptly stops or significantly reduces their intake. Symptoms, as defined by ICD-11, include autonomic hyperactivity (sweating, tachycardia, hypertension), tremors, nausea, insomnia, anxiety, agitation, mood changes, sensory illusions or hallucinations, and distractibility. Severe cases can lead to seizures or delirium.

Signs and Symptoms: These include physical signs like tachycardia, dilated pupils, and sweating; motor symptoms like tremors and seizures; awareness issues like insomnia and agitation; and psychiatric symptoms like hallucinations and paranoia.

Management: The primary treatment for acute alcohol withdrawal is benzodiazepines. Other treatments may include antiemetics for nausea, intravenous fluids, and, in severe cases, parenteral medication. An Essential Guide for Nurses and Paramedics in Custody and Sexual Assault Referral Centres

As we commence Safeguarding Week, it's an opportune moment to reflect on our responsibilities and enhance our understanding of safeguarding practices. For nurses and paramedics working in police custody and sexual assault referral centres, recognising and responding to safeguarding concerns is crucial. This blog provides an overview of safeguarding adults and children, different types of abuse, and key issues to be aware of.

Understanding Safeguarding

Safeguarding Adults

Safeguarding adults involves protecting their right to live free from abuse and neglect. This includes any adult (18 years and over) who:

Has needs for care and support.
Is experiencing, or is at risk of, abuse or neglect.
As a result of those needs, is unable to protect themselves from either the risk or experience of abuse or neglect.

The Care Act 2014 sets out six key principles of safeguarding:

Empowerment : Supporting people to make their own decisions.
Prevention : Acting before harm occurs.
Proportionality : Responding appropriately to the risk presented.
Protection : Providing support and representation.
Partnership : Working with local agencies.
Accountability : Transparency in safeguarding practices.

Safeguarding Children

Safeguarding children involves protecting individuals under 18 from abuse, neglect and exploitation. The legal framework includes several key pieces of legislation, such as the Children Act 1989 and 2004, which emphasise the paramount importance of a child's welfare.

Types of Abuse

Physical Abuse

This includes hitting, slapping, pushing, and misuse of medication. In both adults and children, signs may include unexplained injuries, frequent hospital visits, or a history of repeated injuries.

Sexual Abuse

Sexual abuse involves forcing someone to take part in sexual activities without their consent (or too young to consent). It includes contact and non-contact activities. Indicators might be unexplained bruises, changes in behaviour, or sexually transmitted infections.

Emotional Abuse

Emotional abuse involves causing psychological harm through verbal abuse, threats, and intimidation. Signs include low self-esteem, anxiety, depression and withdrawal.

Neglect

Neglect is the failure to meet basic needs, including food, shelter and medical care. Indicators include poor hygiene, malnutrition, untreated medical issues and frequent absences from school.

Financial Abuse

Financial abuse involves the illegal or unauthorised use of a person’s money or property. Signs include sudden changes in financial situation, unpaid bills and unexplained withdrawals.

Self-Neglect

Self-neglect is a form of neglect where an individual fails to attend to their basic needs, such as personal hygiene and health. Indicators include poor personal hygiene, untreated medical conditions and living in unsafe conditions.

Modern Slavery

Modern slavery encompasses human trafficking, forced labor and domestic servitude. Indicators include poor living conditions, few personal possessions and the inability to travel freely.

Domestic Violence and Abuse (DVA)

DVA includes psychological, physical, sexual, financial, and emotional abuse by an intimate partner or family member. Recognising DVA involves noticing signs such as unexplained injuries, changes in behaviour and frequent absences from work or social activities.

Key Issues in Safeguarding

Contextual Safeguarding

Contextual safeguarding involves understanding and responding to young people’s experiences beyond their families, such as in their communities, schools and online. Recognising extra-familial threats is crucial, as abuse can occur in various contexts.

Radicalisation

Radicalisation refers to the process by which individuals come to support terrorism. Vulnerable individuals may be targeted by extremists and signs include changes in behaviour, expressing extremist views, and social isolation. The Prevent strategy is a key part of safeguarding in this area.

Information Sharing

Effective information sharing is crucial in safeguarding and overrides all other considerations if the child or adult is at risk of significant harm. The Data Protection Act 2018 supports the sharing of information to protect individuals at risk.

Actions for Healthcare Professionals

Recognise

Be aware of the signs and symptoms of abuse, neglect, and exploitation. Maintain a high level of vigilance and professional curiosity in your interactions with individuals in custody or sexual assault referral centres.

Respond

Take immediate action when you suspect abuse. This includes providing support, documenting concerns, and following organisational protocols for safeguarding.

Record

Maintain accurate and detailed records of any safeguarding concerns. Document observations, actions taken, and decisions made.

Refer

Make referrals to appropriate safeguarding authorities or services. Follow local referral pathways and ensure that all relevant information is included.

Collaboration

Work in partnership with other agencies and professionals to ensure a coordinated response to safeguarding concerns. This includes sharing information and participating in multi-agency meetings.

Essential Insights for Nurses and Paramedics in Custody and Sexual Assault Referral Centres

As we mark Diabetes Week, it's crucial for healthcare professionals working in police custody and sexual assault referral centres to deepen their understanding of diabetes management. This knowledge is not only vital for providing optimal care but also for ensuring the safety and well-being of individuals with diabetes.

Understanding Diabetes: Type 1 and Type 2

Type 1 Diabetes

Type 1 diabetes, formerly known as insulin-dependent diabetes mellitus (IDDM), is an autoimmune condition where the body attacks insulin-producing beta cells in the pancreas. This results in little to no insulin production. Individuals with Type 1 diabetes must manage their condition with insulin therapy to maintain blood glucose levels. This type often manifests in childhood or adolescence but can occur at any age.

Type 2 Diabetes

Type 2 diabetes, formerly non-insulin-dependent diabetes mellitus (NIDDM), is characterised by insulin resistance and a relative lack of insulin. The pancreas initially produces insulin, but the body's cells do not use it effectively. Over time, the pancreas cannot keep up with the increased demand, leading to elevated blood glucose levels. Type 2 diabetes is more common in adults but is increasingly diagnosed in younger populations due to rising obesity rates.

Key Differences:

Onset: Type 1 is sudden and often diagnosed in younger individuals, while Type 2 develops gradually and is typically diagnosed in adults.
Insulin Production: Type 1 results in no insulin production; Type 2 involves insulin resistance and eventual inadequate insulin production.
Management: Type 1 requires insulin therapy; Type 2 can often be managed with lifestyle changes, oral medications, and sometimes insulin.

Insulin Regimens: Tailored Approaches

For those with diabetes in custodial settings, understanding insulin regimens is essential. Insulin therapy varies based on individual needs and the type of diabetes.

Rapid-Acting Insulin

This type of insulin begins to work within 15 minutes of injection, peaks around 1 hour, and continues to work for 2-4 hours. It is typically used before meals to control postprandial blood glucose spikes. Examples include insulin lispro, insulin aspart and insulin NovoRapid.

Long-Acting Insulin (Basal Insulin)

Long-acting insulins, or basal insulins, are designed to maintain blood glucose levels steady over a long period, mimicking the body’s natural insulin release. They begin to work several hours after injection and last for up to 24 hours or more. Examples include insulin glargine.

Basal-Bolus Regimen

A basal-bolus regimen combines both rapid-acting and long-acting insulins. The long-acting insulin provides a baseline level of insulin throughout the day and night, while the rapid-acting insulin covers mealtime glucose spikes. This approach closely mimics the body’s natural insulin pattern and offers flexible control over blood glucose levels.

Practical Considerations in Custody

Managing diabetes in a custodial setting presents unique challenges. Here are key points for healthcare professionals:

Blood Glucose Monitoring: Regular monitoring is crucial. Ensure that custody facilities are equipped with reliable glucose meters and ketone testing strips. Baseline blood glucose levels should be obtained as soon as possible.
Insulin Administration: Supervise or instruct the self-administration of insulin. If usual insulin is unavailable, secure an appropriate prescription promptly.
Hypoglycaemia Management: Custody staff should be trained to recognise signs of hypoglycaemia, such as weakness, dizziness, confusion, and loss of consciousness. Immediate administration of glucose is vital.
Hyperglycaemia and Ketoacidosis: Elevated blood glucose levels and ketones indicate a medical emergency. Symptoms of diabetic ketoacidosis (DKA) include nausea, vomiting, abdominal pain, and deep laboured breathing. Immediate hospital transfer may be necessary.
